Now that we got some housekeeping knocked out... I found the issue with the upload order files plugin where the new field added to the addresses table needed to be updated for the current database structure. I updated that extension a few minutes ago (4.0.3) so you should be able to download it from your downloads section in your account. Please let me know if this resolved the issue for you so I can close this ticket out.
